Children with Medical Handicaps (CMH)
The Children with Medical Handicaps (CMH) Program is a health care program supported by the Ohio Department of Health (ODH).
CMH links families of children with special health care needs to a network of quality providers and helps families obtain payment for the services their children need.
The CMH mission is to assure, through the development and support of high quality, coordinated systems, that children with special health care needs and their families obtain comprehensive care and services that are family centered, community based and culturally sensitive.
How Can CMH Help?
1. Diagnostic Program
-Children can receive services for up to 6 months to rule out or diagnose a special health care need or establish a treatment plan.
2. The Treatment Program
-Children can receive services for 1 year for treatment of an eligible condition.
3. The Service Coordination Program
-This program helps families locate and coordinate services for their child.
Eligibility Requirements:
-Ages 0-21
-Permanent residents of Ohio
-Under the care of CMH-approved doctor (MD or DO) and have or may have a chronic medical condition
