Official Notice: Unauthorized Third-Party Vendors

 

The Cincinnati Health Department and Cincinnati Vital Records only partners with VitalChek. We do not authorize or partner with any other outside companies to process birth or death certificate orders. The only expedited services are available through VitalChek. To ensure your personal information remains secure and your request is processed, please use only the verified methods that we have provided to avoid excessive fees and significant delays. Currently Ohio is an open state, and no identification is required to obtain a death certification. 

 

 

  • Non-refundable $3 search fee for certificates not found. 
  • Certificates ordered in person or online may be picked up same-day: 8 a.m.-3:45 p.m. 
  • Certificates not picked up by 3:45 p.m. will be placed in the mail within one to two business days.
  • We are not responsible for any postal delays, nor will a certificate be replaced if lost/stolen/damaged during the delivery process.
