There are several steps a business must take to successfully submit a bid or proposal with the City of Cincinnati. Economic Inclusion's specific bid and proposal requirements are detailed below, however we encourage folks to review our video with the Office of Procurement about how to do business with the City. Economic Inclusion Bid & Proposal Forms
Required Forms
- MBE and WBE Participation Commitment Package Coversheet (MWP‐10): This form provides Economic Inclusion with information about the proposed bidder and the advertised goals.
- MBE/WBE Participation Affidavit (MWP‐20): This form provides Economic Inclusion with the bidder's plan to meet or exceed the advertised inclusion goals and the total contract amount.
- MBE/WBE/SBE Subcontractor Utilization Plan (Form 2003): This form tells us about all subcontractors (both certified and non-certified) the bidder intends to use on the project.
Forms Required in Specific Situations
- Statement of Intent to Self-Perform (Form 2004-1): This form is required when the bidder is a city-certified firm and intends to self-perform work to count towards the contract's inclusion goals.

- Pre-Award Participation Waiver/Reduction Request Form (Form MWP-30): This form is required when the bidder is unable to procure sufficient participation from city-certified firms to fulfill the advertised inclusion goals.
